Documents required for the processing of a claim

Unemployment: if employment is suddenly lost:

  • Certificate from the State Employment Agency confirming unemployment status,
  • Certificate from the State Social Insurance Agency regarding unemployment benefit allocation,
  • Copy of the employment contract,
  • Agreement on termination of employment,
  • Invoices, payment documents.

Incapacity for work: if temporarily unable to work due to illness or accident:

  • Medical certificate confirming illness,
  • A and B incapacity sheets,
  • Invoices, payment documents.
